Subject: Additional Information for AHPRA Case Ref: 12809802 – Undisclosed Findings on CT Temporal Bones


Dear Ms. Blomfield,


Thank you for your ongoing attention to my case.


I am writing to submit additional information relevant to my concerns about medical negligence. I recently reviewed the radiology report from a CT Head and Temporal Bones scan performed previously, and I was deeply distressed to discover that critical findings were not disclosed to me by the responsible medical practitioner.


The report identifies bilateral superior semicircular canal dehiscence—a structural defect involving the small bones of the inner ear—as well as mild increased soft tissue density superficial to the right mastoid air cells. Despite the presence of these findings, I was never informed of them at the time, nor was appropriate follow-up or specialist referral arranged.


This omission has caused me great stress and confusion over the past year, as I continued to experience debilitating neurological and vestibular symptoms with no clear explanation or management plan. Discovering that a potential cause was documented but withheld from me has only deepened my trauma and sense of betrayal by the medical system.


The failure to communicate these findings constitutes, in my view, a serious breach of professional duty and standard of care. It also raises concerns about how many other reports may have been similarly disregarded or mismanaged.


I respectfully request that this failure be added to the scope of my ongoing complaint under reference number 12809802. I have attached the full CT scan for your reference.
