Dear Danielle
I am sorry you decided to withdraw from Mental Health care and that the referrals you and I made to Nurse Navigators have been declined. I am also sorry for the delay in getting back to you, I wanted to gather as much information as possible to give you a way forward with your healthcare concerns.
It is my understanding from the staff I have spoken to on your behalf, that your General Practitioner, Dr is a very good General Practitioner (GP). I encourage you to take this email to an appointment with Dr and work through the suggested actions with him.
Mental Health care
Mental Health have advised me they will keep your current referral open and they would welcome you back to their service. The Clinical Director, Dr Luthra has offered to see you and provide a second opinion about the care you are being provided by Mental Health. It is really important that you engage with this service and attend all scheduled appointments. Please confirm to me that you will see Dr Luthra by return email and I will arrange with Mental Health to follow up with you and book the appointment with her.
Mental Health Independent Patient Rights Advisor
I was able to speak to the IPRA, . She has offered to attend the appointment with Dr Luthra with you and I have scanned the brochure xxx gave me about the support an IPRA can provide to clients. Xxx is available on 49xx xxx
Rheumatology Referral
Our Rheumatology service is provided by Royal Brisbane and Women’s Hospital (RBWH). I have spoken to the Nurse Unit Manager of the Rockhampton Outpatient Department. She has recommended you see your GP and seek a new referral with new information, such as blood tests showing abnormal results relevant to Rheumatology which can be assessed by the RBWH Rheumatologists. It will be their decision as to when you will be seen.
Pain Clinic
You were discharged from the RBWH Pain Clinic in 2023, they recommended you be referred for a Nerve Conduction Study. Please ask Dr to refer you for the Nerve Conduction Study and once the results are available, Dr can make a new referral to the RBWH Pain Clinic if he determines it is clinically appropriate.
Nurse Navigator Service
I am sorry the referral to the Nurse Navigator Service has been declined. You have asked what the criteria are for Nurse Navigator referral acceptance and why you were declined. I have sought advice from the Manager of the service and she provides the following advice about the criteria for Nurse Navigators, a person with:
1. A lack of connection to a General Practitioner
2. Needing referrals into a range of medical specialties
3. A complexity of admissions requiring navigation assistance
4. Low health literacy.
The referrals made by you and I were declined for the following reasons:
1. You are well connected with a good GP, who is working with you to manage your conditions and you are connected to the Mental Health team (at the time the referral was considered)
2. You have active referrals to both the Rheumatology Service and to Medicine, if you feel your conditions have worsened since the first referral was made, you can ask your GP to re-refer and escalate with new clinical information (blood test results, imaging, physical capacity). Nurse Navigators can’t do this for you.
3. If your pain has worsened and you are unable to see Medical Clinic regarding pain, please contact your GP and ask him to refer you to the RBWH Pain Clinic.
4. You have a good understanding of the health system and can navigate it.
