Queensland Health

Thank you Ashley but I have done this multiple times already.

I am holding the Head of Queensland Health responsible for all the failures.

I am submitting my complaint to the International Criminal Court on the 1st of July for the Systematic Administrative failure across the State, the Country & World because this is a Crime against humanity, millions of us children have been deprived our fundamental human rights caused by a Multinational corporation & the governments who are elected to represent us.

This escalation on your behalf is meaningless, please refer my complaint higher not lower.

Hi Danielle,

Thanks for your additional email.

As your treatment was at Rockhampton Hospital, Central Queensland Hospital and Health Service (CQHHS), it would remain appropriate for CQHHS to consider in the first instance.

I understand from your email you have reported to CQHHS previously. Are you able to advise who your concerns were raised with? Should you not be satisfied with their response, there would likely be internal escalation points to consider your concerns within the health service.

Following my email yesterday, I have been advised that the Director Quality and Safety in CQHHS is progressing your concerns. I anticipate they will make contact with you in due course.

Subject: Urgent Concern: Inadequate Support and Documentation of Requests – Appointment Tomorrow

Dear Ashley,

I have formally submitted my concerns in writing to Dr Lewis, and I have prepared a checklist for my follow-up appointment scheduled for tomorrow.

However, I was informed today by a nurse that no Social Worker will be available for me at 2pm, despite my repeated requests for this support.

What is especially troubling is that there is no record of my requests for a Social Worker in the notes, even though I have raised this matter on multiple occasions. This failure to document and respond appropriately is not only negligent—it is actively contributing to my distress and exacerbating my symptoms of trauma-related stress.

Given the sensitivity of my case and the broader legal and medical issues I am navigating, I urge your office to:

Investigate the lack of proper record-keeping and follow-through.

Ensure that appropriate mental health support is available to me during and after tomorrow’s appointment.

Acknowledge and address how this oversight impacts my wellbeing, particularly in light of my diagnosed conditions.

This issue forms part of my broader formal complaints regarding systemic failures in duty of care, and it is being included in both domestic and international legal submissions currently in progress.

I respectfully request that you escalate this matter to ensure that appropriate care and documentation processes are urgently restored.
